PHYSICAL ACTIVITY WAIVER AND RELEASE

By participating in classes at FIYAH Movement Wellness, Participants agree to the following statements:

1. I understand that FIYAH Movement Wellness records all online and in person classes, and that if I do not want my image, voice, or representation to be included in or distributed as part of class recordings, I will turn off my video or audio prior to participating in online classes so that I am not recorded. I further understand that recorded classes may be distributed in whole or in part for marketing, promotional, or in support of other business activities.

2. I am voluntarily participating in Yoga, Dance, Fitness, Meditation & Wellness instruction offered during online classes, during which I may receive information and instruction about Dance Fitness and health. I recognize that FIYAH Movement Wellness classes require physical exertion that may be strenuous and may cause physical injury, emotional and mental impact and I am fully aware of what is involved.

3. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in Yoga Dance Fitness Meditation Instruction. I warrant that I am aware of any medical condition that I may have, which may affect my participation, and I agree to fully disclose this information to FIYAH Movement Wellness and its teachers.

4. In consideration of being permitted to participate in Yoga Dance Fitness Meditation Instruction, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I or my property might incur as a result of participation in the program.

5. In further consideration of being permitted to participate in Yoga, Dance, Fitness, Meditation Instruction, I knowingly, voluntarily and expressly waive any claim I may have against FIYAH Fit and its teachers and students for injury or damages that I may sustain as a result of participation in the program.

6. I, my heirs, or legal representatives forever release, waive, discharge, and covenant not to sue FIYAH Movement Wellness with PHOENIX or any of its representatives, owners or teachers for any injury or death caused by their negligence or other acts.

All participants in the online offerings provided by the FIYAH Movement Wellness with PHOENIX have read the above release and waiver of liability and fully understand its contents. They voluntarily agree to the terms and conditions stated above.
